            <title>Resumen</title>
            <p>La melatonina, es una hormona sintetizada principalmente por la glándula pineal, su secreción tiene un ciclo circadiano definido y presenta valores séricos altos durante la noche y casi indetectables durante el día. En humanos este ciclo depende fundamentalmente de factores postraduccionales como la ubiquitinación y proteólisis de la enzima arilaquilamina N-acetiltransferasa. Durante años se consideró esta glándula como la única fuente endógena de melatonina. Actualmente, se conoce que es sintetizada también por la retina, el tracto gastrointestinal, la piel, las plaquetas, la medula ósea, el hígado y otras células. Existen dos tipos de receptores melatoninérgicos descritos en humanos, los cuales están distribuidos en múltiples sistemas orgánicos. Se ha descrito el rol que cumple la melatonina como regulador del tránsito gastrointestinal y como contribuyente al estado de euglucemia. También funge como agente antioxidante en el contexto del infarto agudo del miocardio y la encefalopatía hipóxica isquémica. Se cuenta con varios agonistas para esta molécula con indicaciones específicas. En este momento, existen múltiples estudios clínicos que buscan ampliar los horizontes terapéuticos de la melatonina. </p>

            <title>Abstract</title>
            <p>Melatonin is a hormone synthetized mainly by the pineal gland. Its secretion has a defined circadian rhythm and it presents high serum values during the day but almost undetectable at night. This circadian rhythm in humans depends on postranslational regulation as ubiquitination and aralkylamine N-acetyltransferase´s proteolysis. For several years this gland was considered the only endogenous source of melatonin. Nowadays it is well known that melatonin is produced by the retina, gastrointestinal tract, skin, platelets, bone marrow, liver and other cells. There are two types of melatonin receptors, which are distributed in multiple organs. The role of melatonin as a regulator of gastrointestinal transit and as a contributor to the status of euglycemia has been described. It also acts as an antioxidant agent, in the context of acute myocardial infarction and hypoxic ischemic encephalopathy. Currently there are several melatonergic agonists with specific indications. At this time there are multiple clinical trials that seek to expand the therapeutic horizons of melatonin.</p>

         <title>INTRODUCCIÓN</title>
         <bold> </bold>
         <p>La melatonina o N-acetil-5-metoxitriptamina es una neurohormona derivada del triptófano [<xref ref-type="bibr" rid="BNaN">1</xref>,<xref ref-type="bibr" rid="BNaN">2</xref>]. Fue descubierta y aislada en la glándula pineal bovina por Aaron Lerner en 1958 [<xref ref-type="bibr" rid="BNaN">3</xref>]. Según la teoría de endosimbiosis, se presume que la capacidad de síntesis de melatonina ha evolucionado por aproximadamente 2.5 billones de años. Inicialmente, era sintetizada por bacterias procariotas Rhodospirillum rubrum [<xref ref-type="bibr" rid="BNaN">4</xref>], las cuales fueron fagocitadas por células eucariotas donde evolucionaron a mitocondrias [<xref ref-type="bibr" rid="BNaN">5</xref>]. Se especula que todas las mitocondrias de células eucariotas tienen la capacidad de sintetizar melatonina. A pesar de este proceso evolutivo, la melatonina no ha sufrido modificaciones en su estructura química [<xref ref-type="bibr" rid="BNaN">6</xref>,<xref ref-type="bibr" rid="BNaN">7</xref>].</p>
         <p>Actualmente, ha resurgido el interés en la investigación sobre esta hormona debido a su función crítica en la regulación de múltiples sistemas biológicos, así como por sus posibles </p>
         <p>beneficios farmacológicos. La melatonina regula el ciclo circadiano [<xref ref-type="bibr" rid="BNaN">8</xref>], el desarrollo puberal, el sistema inmune [<xref ref-type="bibr" rid="BNaN">9</xref>], el sistema cardiovascular [<xref ref-type="bibr" rid="B0">10</xref>,<xref ref-type="bibr" rid="B1">11</xref>], el control del daño oxidativo [<xref ref-type="bibr" rid="B2">12</xref>] y el envejecimiento. Se han descrito efectos antiinflamatorios, antitumorales, antioxidantes y analgésicos asociados al uso de melatonina [<xref ref-type="bibr" rid="B3">13</xref>,<xref ref-type="bibr" rid="B4">14</xref>].</p>
      </sec>
      <sec>
         <title>REGULACIÓN DE LA SECRECIÓN</title>
         <bold> </bold>
         <p>La melatonina, es una molécula altamente lipofílica y no es almacenada de forma intracelular posterior a su síntesis; por esto existe una relación estrecha entre la síntesis y los niveles circulantes de la hormona [<xref ref-type="bibr" rid="B5">15</xref>]. Esta es secretada por la glándula pineal [<xref ref-type="bibr" rid="B6">16</xref>]. La cual es una estructura encefálica derivada de la porción caudal del diencéfalo y se encuentra fuera de la barrera hemato-encefálica [<xref ref-type="bibr" rid="B7">17</xref>]. Durante años se consideró como la única fuente endógena de melatonina. Actualmente, se considera que es sintetizada por la retina, el tracto gastrointestinal, la piel, las plaquetas, la medula ósea, el hígado y otras células [<xref ref-type="bibr" rid="B3">13</xref>].</p>
         <p>En la glándula pineal, la melatonina presenta un ritmo de secreción circadiano, con mayores niveles nocturnos y niveles basales casi indetectables durante el día [<xref ref-type="bibr" rid="B6">16</xref>]. En humanos la secreción inicia posterior al atardecer y alcanza un pico máximo entre las 2:00-4:00 h (20-22 Circadian Time) [<xref ref-type="bibr" rid="B8">18</xref>]. Sin embargo, a diferencia de la síntesis en la glándula pineal, los otros órganos no presentan un ritmo circadiano de producción de melatonina, ni la secretan en cantidades considerables al torrente sanguíneo, [<xref ref-type="bibr" rid="B6">16</xref>] sino que, ejercen sus efectos actuando a nivel paracrino [17].</p>
      </sec>
      <sec>
         <title>REGULACIÓN DE LA SÍNTESIS</title>
         <bold> </bold>
         <p>La glándula pineal recibe estímulos principalmente del núcleo supraquiasmático (NSQ), y del tracto retino hipotalámico [<xref ref-type="bibr" rid="B9">19</xref>]. Este tracto inicia con las células ganglionares fotorreceptoras intrínsecas de la retina, las cuales contienen el fotopigmento melanopsina, que capta luz con longitud de onda de 480 nm [<xref ref-type="bibr" rid="B0">20</xref>]. Estas neuronas proyectan sus axones hacia el núcleo supraquiasmático ventrolateral, donde al despolarizarse, liberan glutamato y polipéptido activador de la adenilato ciclasa de la pituitaria (PACAP). Las neuronas del NSQ liberan ácido γ-aminobutírico (GABA) en la sinapsis con el núcleo paraventricular del hipotálamo, lo que genera que disminuya su función [<xref ref-type="bibr" rid="BNaN">8</xref>]. De este núcleo, se proyecta el haz medial del cerebro anterior hacia la columna celular intermedio lateral, de la lámina VII de Rexed entre los niveles T1 y T3 de la médula espinal. Asimismo, dentro de dicho nivel se encuentra la neurona preganglionar la cual, hace sinapsis a nivel del ganglio cervical superior con la neurona posganglionar. Esta sinapsis es de tipo excitatoria vía acetilcolina. De estos ganglios se originan los nervios pineales, los cuales son nervios simpáticos noradrenérgicos.  En ausencia de luz el NSQ libera menor cantidad de GABA sobre el núcleo paraventricular, por lo tanto hay menos inhibición (<xref ref-type="bibr" rid="B1">21</xref>,<xref ref-type="bibr" rid="B2">22</xref>).</p>

         <p>El  triptófano es captado del plasma por medio de un sistema de aminoácidos aromáticos de alta afinidad. Luego es hidroxilado por la enzima triptófano hidroxilasa, por medio de la enzima L-aminoácido decarboxilasa es convertido a serotonina. La serotonina es acetilada por la enzima arilaquilamina N-acetiltransferasa (AANAT) a N-acetilserotonina. Finalmente es convertida a melatonina por la enzima hidroxindol-o-metiltransferasa (HIOMT) [<xref ref-type="bibr" rid="B3">23</xref>,<xref ref-type="bibr" rid="B4">24</xref>].</p>
         <p>La regulación de la síntesis de melatonina ha sido ampliamente estudiada y fue descrita inicialmente en roedores. Originalmente, se propuso que la única enzima limitante en la síntesis de la melatonina era la AANAT, por lo que la actividad de la enzima regulaba la producción rítmica de melatonina [<xref ref-type="bibr" rid="B5">25</xref>]. En la figura No. 2 se ejemplifica el proceso de síntesis de melatonina a partir de la estimulación simpática. </p>
         <p>Se ha propuesto que en la glándula pineal de ratas existe modulación por nervios parasimpáticos que se originan del ganglio pterigopalatino (Ver Figura. No. 3) [<xref ref-type="bibr" rid="B7">27</xref>,<xref ref-type="bibr" rid="B8">28</xref>]. En humanos no se ha descrito una inervación parasimpática de la glándula pineal propiamente dicha. Sin embargo, se han descrito grupos de interneuronas intrapineales con actividad acetilcolinesterasa. Además, se han propuesto los ganglios de Marburg y Pastori, que están localizados sobre la glándula pineal, como parte de un sistema pineal parasimpático [<xref ref-type="bibr" rid="B7">27</xref>,<xref ref-type="bibr" rid="B8">28</xref>].</p>

         <p>(1) La norepinefrina (NE) es liberada desde el ganglio cervical superior y se une a su receptor β1-adrenérgico en el pinealocito [<xref ref-type="bibr" rid="B6">26</xref>]. </p>
         <p>(2) La actividad β1 genera un aumento de adenosín monofosfato cíclico (AMPc) [<xref ref-type="bibr" rid="B6">26</xref>].</p>
         <p>(3)  Este aumento de AMPc genera mayor actividad de Proteín Quinasa A (PKA), la cual fosforila a la enzima AANAT [<xref ref-type="bibr" rid="B6">26</xref>].</p>
         <p>(4) El AMPc se trasloca al núcleo y fosforila la proteína de unión al elemento de respuesta al AMPc (CREB, por sus siglas en inglés), el cual funciona como promotor en el gen de AANAT. Esto se ha observado de manera in vitro [26].</p>
         <p>(5) La NE se une también a receptores α-adrenérgicos en el pinealocito [<xref ref-type="bibr" rid="B6">26</xref>]. </p>
         <p>(6) La acción α-adrenérgica estimula la fosfolipasa-C (PLC), que produce un aumento en las concentraciones de calcio iónico intracelular. </p>
         <p>(7) El aumento del calcio iónico estimula la Proteín Quinasa Dependiente de Calcio (PKC) [<xref ref-type="bibr" rid="B6">26</xref>].</p>
         <p>(8) La acción de PKA y PKC aumenta la actividad de la AANAT [<xref ref-type="bibr" rid="B3">23</xref>,<xref ref-type="bibr" rid="B4">24</xref>].</p>
         <p>(9) La serotonina entra al pinealocito por medio de un transportador activo para aminoácidos aromáticos. Esta es el precursor de la N-acetilserotonina, que es sustrato de la AANAT (Ver figura. No. 1) [<xref ref-type="bibr" rid="B3">23</xref>,<xref ref-type="bibr" rid="B4">24</xref>].</p>
         <p>(10) La HIOMT cataliza el paso final de la síntesis de melatonina [<xref ref-type="bibr" rid="B3">23</xref>,<xref ref-type="bibr" rid="B4">24</xref>]. </p>

         <p>(1) Se libera ACh sobre receptores nicotínicos, generando despolarización de la membrana [<xref ref-type="bibr" rid="B7">27</xref>,<xref ref-type="bibr" rid="B8">28</xref>]. </p>
         <p>(2) La despolarización abre canales de Calcio Cav aumentando la concentración de calcio iónico intracecular [<xref ref-type="bibr" rid="B7">27</xref>,<xref ref-type="bibr" rid="B8">28</xref>]. </p>
         <p>(3) El calcio se une a las vesículas de glutamato, lo que favorece su liberación [<xref ref-type="bibr" rid="B7">27</xref>,<xref ref-type="bibr" rid="B8">28</xref>]. </p>
         <p>(4) El glutamato puede actuar de forma autocrina o paracrina, al unirse a receptores de glutamato tipo 3 (GLU3), los cuales se acoplan a proteínas Gi [<xref ref-type="bibr" rid="B7">27</xref>,<xref ref-type="bibr" rid="B8">28</xref>].</p>
         <p>(5) La actividad Gi inhibe la adenilato ciclasa, lo que reduce los niveles intracelulares de AMPc. </p>
         <p>(6) Esta disminución en la síntesis de AMPc inhibe la AANAT, la fosforilación de la AANAT y la síntesis de esta enzima desde el ADN [<xref ref-type="bibr" rid="B7">27</xref>,<xref ref-type="bibr" rid="B8">28</xref>].  </p>
         <p>Sin embargo, en humanos la regulación de la síntesis de melatonina no está bien descrita, se ha propuesto que la regulación de la enzima AANAT no se da a un nivel transcripcional, como se describió en roedores. En humanos depende más de factores postraduccionales. A pesar de que se ha demostrado una diferencia entre los niveles de actividad diurna y nocturna de la AANAT, los niveles de ARNm de AANAT y de HIOMT se mantienen constantes [<xref ref-type="bibr" rid="B9">29</xref>]. Esto explica en parte la dinámica de la actividad de la enzima en humanos, la cual aumenta casi inmediatamente con el cese del estímulo luminoso, lo que se opone a la dinámica lenta del proceso de transcripción [<xref ref-type="bibr" rid="B0">30</xref>]. Cuando se suspende el estímulo de NE, el CREB fosforilado disminuye hasta niveles basales en aproximadamente una hora, probablemente por la acción de fosfatasas [<xref ref-type="bibr" rid="B4">24</xref>].</p>
         <p>Se han propuesto la ubiquitinación y la proteólisis de la enzima AANAT como factores reguladores postraduccionales de la síntesis de la melatonina [<xref ref-type="bibr" rid="B1">31</xref>]. El sistema de ubiquitinación de proteínas es particularmente importante en las enzimas que tienen vidas medias cortas. Consta de tres reacciones enzimáticas consecutivas, las cuales involucran una enzima activadora de ubiquitinación (E1), una enzima conjugadora de ubiquitinación (E2), y una enzima con actividad ubiquitín ligasa (E3). Una vez finalizado este proceso, la proteína es degradada por el proteosoma. Durante la noche el aumento de la actividad de la PKA fosforila la proteína 14-3-3 delta/épsilon, que al estar fosforilada se une a AANAT y evita que sea degradada por el proteosoma [<xref ref-type="bibr" rid="B2">32</xref>]. Además, esta unión aumenta la afinidad de la AANAT por sus sustratos, lo que es de especial importancia durante la noche, cuando los niveles de serotonina en la glándula pineal disminuyen hasta 10 veces comparados con los niveles diurnos por su consumo en la reacción [<xref ref-type="bibr" rid="B3">33</xref>]. </p>
      </sec>
      <sec>
         <title>RECEPTORES DE MELATONINA</title>
         <bold> </bold>
         <p>Existen dos receptores de melatonina conocidos en humanos. En la tabla No. 1 se establecen las características de los receptores de melatonina. Al activar ambos receptores con análogos de melatonina se describió una disminución de los niveles de AMPc intracelular, mediado por una proteína Gi sensible a toxina Pertussis [<xref ref-type="bibr" rid="B7">37</xref>,<xref ref-type="bibr" rid="B8">38</xref>]. Se demostró también que a través de la activación de estos se puede estimular la Fosfolipasa C (PLC, por sus siglas en inglés) [<xref ref-type="bibr" rid="B9">39</xref>], así como la actividad de la Quinasa c-Jun N-terminal (JNK) vía proteína G16 o vía Gi sensible a toxina Pertussis. La JNK es una proteína que pertenece al grupo de Quinasas Activadas por Mitógenos (MAPK, por sus siglas en inglés). Esto sugiere que la melatonina regula la expresión génica de múltiples proteínas al estimular esta quinasa [<xref ref-type="bibr" rid="B0">40</xref>]. </p>

         <p>En el sistema nervioso central se describió la presencia del Receptor de Retinoides X (RXR), principalmente en la glándula pineal, NSQ, tálamo e hipotálamo, entre otros. Este receptor forma parte de la subfamilia de receptores nucleares huérfanos, los cuales al ser estimulados por ligandos inducen factores de transcripción. Por otra parte, se demostró que la melatonina puede estimular el RXR-β y de esta forma modular la transcripción génica [<xref ref-type="bibr" rid="B1">41</xref>]. Un ejemplo de esto es la regulación a la baja que genera la melatonina en la expresión de 5-lipooxigenasa en linfocitos tipo B [42].</p>
      </sec>
      <sec>
         <title>FARMACOLOGÍA</title>

         <p>Medicines Agency (EMA) para su venta y distribución como suplemento alimenticio [<xref ref-type="bibr" rid="B3">43</xref>]. Se han desarrollado otras moléculas que fungen como agonistas melatoninérgicos, que cuentan con aprobaciones específicas como fármacos. En el cuadro No. 2 se detallan los parámetros farmacológicos de algunas de estas moléculas.</p>
      </sec>
      <sec>
         <title>USOS TERAPÉUTICOS</title>
         <bold> </bold>
         <p>En múltiples estudios se ha intentado demostrar la utilidad de los agonistas melatoninérgicos en diversos contextos clínicos: </p>
         <p>•	Síndrome de abstinencia por uso de benzodiazepinas (BDZ)</p>
         <p>Este síndrome cursa con un cuadro de hiperexcitabilidad del sistema nervioso central el cual provoca alteraciones en el sueño [48,49]. Se </p>
         <p>ha utilizado melatonina en el tratamiento de pacientes con cuadros de abstinencia por BDZ [<xref ref-type="bibr" rid="B9">49</xref>]. Los estudios no presentan resultados congruentes con respecto a si la melatonina disminuye la necesidad del uso de BDZ en el manejo del síndrome de abstinencia por BDZ [<xref ref-type="bibr" rid="B8">48</xref>,<xref ref-type="bibr" rid="B9">49</xref>]. A pesar de esto, se recomienda su uso como medida terapéutica en este síndrome, con un grado de evidencia: recomendación de expertos [<xref ref-type="bibr" rid="B0">50</xref>,<xref ref-type="bibr" rid="B1">51</xref>].</p>
         <p>La evidencia muestra que el uso de melatonina mejora la calidad del sueño en aquellos pacientes que lograron descontinuar las BZD, esto se valoró por medio de la escala Pittsburgh Sleep Quality Index (PSQI) de la calidad del sueño. En estudios de polisomnografía, los resultados no mostraron diferencia entre los sujetos a los que les administró melatonina contra los que se les administró placebo para discontinuar el uso de BDZ [<xref ref-type="bibr" rid="B2">52</xref>,<xref ref-type="bibr" rid="B3">53</xref>].</p>
         <p>Algunas posibles causas por las cuales no se han obtenido resultados congruentes están en relación con: el desconocimiento de la forma óptima de regular la dosis de melatonina (niveles séricos o dosis ingerida), y la falta de estandarización del tipo de fármaco óptimo (de liberación prolongada o de acción inmediata) de acuerdo patologías específicas, entre otros factores [<xref ref-type="bibr" rid="B1">51</xref>]. </p>
         <p>•	Gastrointestinal </p>
         <p>La melatonina tiene la capacidad de producir contracción o relajación en el músculo liso del tracto gastrointestinal. Su función relajante estudiada en el músculo liso del fondo gástrico en ratas, se debe a la estimulación de canales de potasio dependientes de calcio y a la desensibilización hacia la serotonina [54]. Por otra parte, la melatonina puede inhibir la óxido nítrico sintasa neural, lo que disminuye la liberación del óxido nítrico hacia el músculo liso del colon de cobayo, lo que favorece su contracción [55]. </p>
         <p>Estudios realizados en intestino delgado de rata demuestran que la melatonina es capaz de inducir el Complejo Motor Migratorio (CMM) y reducir la actividad electromiográfica irregular en espigas del intestino. En el mismo estudio se utilizó un bloqueador de receptores de melatonina lo que induce a un comportamiento contrario [56]. De igual forma, se ha descrito que en pacientes con pinealectomía se altera el CMM (este puede aumentarse o disminuirse), y que con suplementación de melatonina se logra reestablecer el patrón usual [55].</p>
         <p>Se demostró que el uso de melatonina es capaz de reducir el dolor abdominal en pacientes con Síndrome de Intestino Irritable (SII) [57]. Estudios clínicos en pacientes con SII y trastornos del sueño concomitante, han demostrado que la suplementación con melatonina aumenta el umbral de presión rectal, induce la sensación de defecar y reduce el dolor abdominal. Se presenta una tendencia hacia la disminución de la frecuencia defecatoria, mejora la consistencia de las heces y los síntomas del síndrome en general [58]. Es importante señalar que en el estudio mencionado no se reportó el espectro predominante del SII de los participantes (diarreico o constipatorio). Los resultados obtenidos pueden ser beneficiosos o perjudiciales para un espectro u otro, por lo que debe considerarse esto a la hora de hacer recomendaciones. </p>
         <p>No se han encontrado receptores para melatonina en músculo liso gastrointestinal, a pesar de su papel regulador descrito en la motilidad intestinal; por lo que se cree que estos receptores de melatonina están en algún elemento neuronal del tracto, lo que también explicaría el efecto analgésico que esta  presenta [55]. </p>
         <p>•	Efecto Antioxidante </p>
         <p>Los radicales libres y el estrés oxidativo se han asociado con gran número de enfermedades, incluidos el cáncer, la aterosclerosis, algunas enfermedades neurodegenerativas, diabetes mellitus, lesiones por isquemia-reperfusión, entre otras [59,60]. Existe evidencia del efecto antioxidante que desempeña no solo la melatonina, sino también los metabolitos derivados de esta [61]. Cuando la melatonina reacciona con el peróxido de hidrógeno forma N1-acetil-N2-formil-5-metoxikinuramina (AFMK), mediante una reacción oxidativa este puede formar N1-acetil-5-metoxikinuramina (AMK), que tiene aún más capacidad de eliminar radicales libres del óxido nítrico que sus precursores [59]. Es debido a esto que, en experimentos in vitro, se observó que la eliminación de radicales libres mediante melatonina es más prolongada que la observada con vitamina C, NADH, NADPH o glutatión [62]. </p>
         <p>La melatonina aumenta la expresión génica y actividad de otras moléculas antioxidantes como por ejemplo: catalasas, glutatión sintasa, glutatión peroxidasa, glutatión reductasa, glucosa-6-fosfato deshidrogenasa y superóxido dismutasa [63,64]. Puede inhibir la expresión de enzimas pro-oxidantes como: el óxido nítrico sintasa y reducir niveles plasmáticos de malondialdehído [65]. La melatonina se sintetiza en grandes cantidades en la mitocondria y se plantea que constituye uno de los factores más importantes en la eliminación de especies reactivas de oxígeno [63]. Recientemente se describió que promueve la mitofagia, esta es una degradación selectiva de la mitocondria mediada por autofagia. Este proceso es crítico para la supervivencia celular, ya que, la acumulación de daño mitocondrial puede generar la liberación de radicales libres de oxígeno que inducen piroptosis vía inflamasoma NLRP3 [66,67].</p>
         <p>Debido a estas acciones la melatonina se ha propuesto como una medida terapéutica a considerar en la prevención de lesiones por isquemia-reperfusión en diferentes tejidos, entre ellos: el cerebro y el miocardio [68, 69, 70]. En un estudio realizado en corazones de ratas que fueron sometidos a isquemia y luego reperfusión, el tratamiento con melatonina disminuyó la incidencia, duración y severidad de las arritmias generadas por este tipo de lesión al miocardio [71]. Además, se documentó que la melatonina en estas células induce regulación a la alta de la vía de señalización JAK2/STAT3, acompañado de un aumento en el factor antiapoptótico Bcl2 y una reducción en el factor proapoptótica Bax [10]. </p>
         <p>En un estudio prospectivo y aleatorizado, se evaluó el uso de 100 mg/kg de peso corporal  de melatonina vía oral por cinco días en neonatos prematuros con encefalopatía hipóxico-isquémica sometidos a protocolo de hipotermia. Se documentó una mejoría en los electroencefalogramas y resonancias magnéticas cerebrales de control; así como menor incidencia de convulsiones en los pacientes que recibieron melatonina en comparación con los que sólo recibieron hipotermia. Sin embargo, la diferencia no fue significativa. No se documentaron efectos adversos con el tratamiento [68].  En otro estudio aleatorizado, se evaluó el uso de una dosis de 20 mg de melatonina vía oral en pacientes con sepsis neonatal, se demostró una disminución significativa en el conteo total de glóbulos blancos, el conteo absoluto de neutrófilos y la proteína C reactiva 24 horas posteriores a la administración de melatonina versus el control. Es importante mencionar que ambos estudios contaron con muestras pequeñas y amplia variabilidad en la dosis y frecuencia de administración de melatonina. Por lo que es necesario ensayos estandarizados y con muestras de mayor tamaño antes de poder emitir recomendaciones con respecto al uso de melatonina como cotratamiento en estas patologías [72].</p>
         <p>•	Diabetes mellitus tipo 2 (DM-2)</p>
         <p>La melatonina ha sido investigada por su posible participación en el metabolismo de los carbohidratos, ya que este también presenta comportamiento circadiano. Estudios demuestran que los receptores para melatonina son en gran medida responsables de la euglicemia [73].</p>
         <p>Se ha logrado establecer relaciones epidemiológicas entre mutaciones del gen MTNR1B, que codifica para receptores de melatonina de tipo MT1 (presente en el cromosoma 11), y poblaciones con alta incidencia de DM-2 e intolerancia a carbohidratos [74-76]. Las mutaciones puntales que han sido establecidas como factores de riesgo para DM-2 son: </p>
         <p>•	Polimorfismo de nucleótido simple (SNP, por sus siglas en inglés) rs10830963: se asocia a glicemia en ayuno elevada y disminuye la sensibilidad a los análogos del péptido similar al glucagón tipo 1 (GLP-1) cuando es utilizado como tratamiento de DM-2 [75,77].</p>
         <p>•	SNP rs2166706: se asocia a niveles altos de hemoglobina glicosilada y reducción de la respuesta homeostática de la célula beta pancreática [78].</p>
         <p>•	SNP rs1387153 y rs11020107: se asocia a glicemia en ayuno elevada [77,79].</p>
         <p>Se han descrito receptores de melatonina de tipo MT1 en el islote pancréatico. Hay modelos animales que establecen que los polimorfismos en el gen MTNR1B pueden provocar que se sintetice un receptor con mayor afinidad hacia proteínas Gi en células alfa y beta pancreáticas. En células alfa aumenta la liberación de glucagón y en la célula beta pancreática al disminuir la actividad de la PKA, reduce la primera fase de liberación de insulina [73,76,80,81]. </p>
         <p>Altas concentraciones séricas de melatonina pueden aumentar el calcio intracelular en la célula beta por medio de PLC, lo que aumenta la secreción de insulina. Es por esto, que se ha planteado la posibilidad de suplementar con melatonina a los sujetos en quienes se haya documentado alguno de estos polimorfismos [74,80].</p>
      </sec>
      <sec sec-type="conclusions">
         <title>CONCLUSIONES</title>
         <bold> </bold>
         <p>A pesar de que esta molécula fue descubierta desde 1958, hoy en día se encuentra en auge de investigación debido a sus múltiples efectos farmacológicos. Se han descrito efectos antiinflamatorios, antitumorales, antioxidantes y analgésicos asociados a su uso. Debido a su importante efecto antioxidante, se ha propuesto como cotratamiento en múltiples enfermedades en las que su fisiopatología se explica por el estrés oxidativo, incluidos el cáncer, la aterosclerosis, algunas enfermedades neurodegenerativas, la diabetes mellitus, lesiones por isquemia-reperfusión, entre otras.</p>
         <p>La falta de estandarización con respecto a la dosis, frecuencia y presentación a utilizar de los agonistas melatoninérgicos, genera que los estudios clínicos publicados hasta el momento sean heterogéneos y con resultados poco comparables. Consideramos que, deben realizarse nuevos estudios con dosis estandarizadas y muestras de mayor tamaño antes de poder recomendar el uso de melatonina como tratamiento para las patologías antes descritas.</p>
      </sec>
      <sec>
